Cognitive Decline, Dementia & Alzheimer’s

As people age, their risk for memory loss increases.  There are many risk factors involved.  Our goal is to reduce the risk factors associated with memory loss as well as to restore as much function as possible to maximize the quality of life for our patients.

Here are the steps:

Step 1: Fill out the paperwork.  It typically takes about 1 hour.  You can download and print the paperwork from our website or come to the clinic for a packet of paperwork. 

We will need:

  1. Welcome Form
  2. Neurotransmitter Assessment Form
  3. Brain Region Localization Form
  4. Brain Function Assessment Form
  5. Metabolic Assessment Form

Also, please give us a list of all your medications and supplements as well as a list of injuries, surgeries, and medical conditions that you have been diagnosed with.

Step 2: Central Nervous System Vital Signs: Cognitive Domain Testing.  We will test and quantify the different aspects of memory.  This typically takes approximately 1 hour.

Step 3:  Quantitative Electroencephalography (QEEG) This also takes approximately 1 hour.

We will have you sit in a chair and we will hook you up to electrodes as you will be wearing a cap.  We will measure your brain wave activity.  This will help us to isolate exactly what areas of the brain to treat later. 

Step 4: We need to order blood work, to understand the biochemistry and physiology of the brain and body, so that we can have successful treatment.  Please give us any blood work that has been already done within the last 12 month period.

Step 5: After we have collected all the data, we will call you for a follow-up.  This will last 30 min. At this time, we will go over your results and formulate a treatment plan.  You can either do things at home or at our clinic or in combination to increase functional capacity and outcome of your brain.

Total cost: exam and follow-up is $400
Labs: Variable (depending on insurance)
Neurofeedback treatments: $150 per session or $1000 prepaid (non-refundable) for 10 sessions.  We don’t re-test until after 20 sessions.

Re-exam cost: $200
Cost to create home program: $84 (30 min. appointment)
